The reassuring news: it is almost always manageable. Here is why it happens, and exactly what you can do about it.
Why does back pain happen during pregnancy?
There are several reasons and they all make complete physiological sense:
As your belly grows, your body's weight distribution changes. You naturally lean back slightly to compensate, which puts additional strain on the muscles and ligaments supporting your lower spine.
Relaxin is doing its job too well
Your body produces a hormone called relaxin during pregnancy, which loosens the ligaments in your pelvis to prepare for childbirth. The problem: it also loosens ligaments throughout the rest of your body including your back making it less stable and more prone to strain.
Many women develop a natural arch in the lower back (called lordosis) as pregnancy progresses. Without mindful posture correction, this leads to significant muscle fatigue and pain.
Muscle separation (Diastasis recti)
Your abdominal muscles are separating to accommodate your growing uterus. These muscles normally support your back as they separate, your back has to work harder.

Types of pregnancy back pain to know
Lumbar pain: The most common type aching in the lower back, made worse by prolonged sitting or standing
Posterior pelvic pain: A deep pain felt lower in the back, near the tailbone often described as shooting pain into the buttocks or thighs
Sciatica: When the sciatic nerve is compressed causes pain, numbness, or tingling running from the lower back down through the buttock and leg. Common in the second half of pregnancy.
How to relieve pregnancy back pain: what actually works
Most pregnancy back pain improves dramatically with posture awareness:
Stand tall with your chest open and shoulders back
When sitting, use a rolled towel or small cushion to support the curve of your lower back
Avoid standing on one hip distribute weight evenly across both feet
When lifting anything (including older children), bend at the knees, not the waist
Sleeping position
Sleep on your left side with a pillow between your knees and another under your belly. This position aligns the spine, reduces pressure on the lower back, and improves blood flow to the baby. A pregnancy pillow (C-shaped or U-shaped) makes this far more comfortable.
Pregnancy pillows are available on Amazon India, Firstcry, and Mothercare India for approximately ₹800–₹3,000. A desi jugaad alternative: stack two regular pillows in a C-shape.
Warm compress
A warm (not hot) water bottle placed on the lower back for 15–20 minutes provides significant relief. Many Indian women use a hot bag or kapde mein bandha namak (warm salt bag) both work well.
Pregnancy-safe exercises
Regular gentle exercise is one of the best treatments for pregnancy back pain. Try:
Cat-cow pose (marjaryasana-bitilasana): Extremely effective for back pain gently moves the spine through its full range. Do 10 repetitions slowly.
Child's pose (balasana): Releases the lower back beautifully. Widen the knees to accommodate your belly.
Pelvic tilts: Standing or on all fours, gently tuck and release the pelvis. Strengthens the core without straining the abdomen.
Swimming: The water supports your weight completely walking or gentle swimming is ideal for back pain relief in pregnancy.

A maternity belt or pregnancy support belt takes the weight of the belly off your lower back. Excellent for women who spend long hours standing or walking. Use for a few hours at a time do not wear it all day as your muscles need to work independently too.
Most pregnancy back pain is musculoskeletal and not dangerous. However, see your doctor immediately if:
Pain is severe, sudden, or getting worse rapidly
Accompanied by fever or burning during urination (could indicate a kidney infection)
Accompanied by regular contractions or vaginal bleeding
You experience numbness or weakness in your legs
